The Folsom Lake College Athletic Training staff provides comprehensive prevention, care, treatment, and rehabilitation services for our student-athletes and visiting student athletes. It is the responsibility of the Athletic Training Staff to evaluate and treat injuries that occur during our athletic practices and contests.

VISITING TEAM INFORMATION: The Athletic Training room is located in PE-102 and an Athletic Trainer will be available at least 1 hour before every athletic contest held at Folsom Lake College for any athletic training services needed by visiting teams. If there are any special services needed (E-Stim, Ultrasound, etc.) please have your athletic trainer contact me directly prior to arriving at Folsom Lake College. Water, cups, injury ice, and a blood kit will be provided to the visiting teams bench. The Athletic Trainer on site will have a full medical kit, AED, and crutches.
2014 Pre-Participation Physical Exam/Clearance The dates for 2014 pre-participation physical exams will be as follows:
Wednesday May 28th at 3:30pm - Men's Soccer Wednesday June 25th at 1:00pm - Women's Soccer - All prospective student athletes MUST contact Athletic Trainer, Mike Green, via email to schedule an email@example.com Wednesday August 6th at 4:00pm - Baseball - All prospective student athletes MUST contact Athletic Trainer, Mike Green, via email to schedule an firstname.lastname@example.org TBA - Men's Tennis, Women's Tennis, Men's Golf
They will take place in the Physical Education building in PE-102. Athletes who come on the day of physicals with incomplete forms will not be seen!
If you cannot attend the dates above for your respective sport or if you prefer, you may obtain a physical exam from your own personal physician (ONLY M.D., D.O., Nurse Practitioner, or Physician Assistant. NO D.C., or D.P.T.). If obtaining a physical from your own physician, please use our "Preparticipation Exam" form and have physician complete page 2 in its entirety.
ALL CURRENT AND POTENTIAL STUDENT ATHLETES MUST SUBMIT THE FOLLOWING DOCUMENTS: 1) 1 WEEK PRIOR TO PHYSICAL DATE (IF PHYSICAL IS DONE AT SCHOOL) 2) 1 WEEK PRIOR TO FIRST PRACTICE (IF PHYSICAL IS DONE BY PERSONAL PHYSICIAN)
